Your First Visit Involves a Few Steps
In order to see Dr. Oshman in Westport or New Canaan, please follow the steps below.
STEP 1: Schedule an Appointment
Many patients come to Dr. Oshman through referral. When you call, please be sure to tell us who referred you to our office. We are grateful for everyone who shares their experience! Please click this link to schedule an appointment or call us at 203-454-0743 (Westport) or 203-972-5909 (New Canaan).
STEP 2: Fill Out New Patient Form(s)
To help expedite your first appointment, please bring completed paperwork with you. Please click this link to download and fill out the forms you may need and bring them with you on your visit.
STEP 3: Come to Your First Appointment
Our office is concerned with your needs. We will schedule an appointment on the same day you call if the need arises. There is usually little or no wait to see Dr. Oshman. We never overbook. If, for any reason, you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible.
Insurance
In order to ensure the best possible care, we are an out of network provider. Patients will receive a receipt of service which can be used when submitting an out of network claim form to his/her insurance company. Qualifications for insurance coverage may differ due to the uniqueness of each procedure and insurance plan. We are happy to write a letter on your behalf to your insurance company to aid in your reimbursement.
Medicare
Although we continue to see Medicare patients, our office has "opted out" of Medicare. This means that Medicare patients may not submit any of our bills to Medicare. If you have a secondary insurer the office will be happy to submit a bill to that insurer. Depending on your secondary policy you may or may not receive partial reimbursement for your visit. Medicare will not reimburse at all for the visit. All Medicare patients must complete the Medicare Contract located at this link.
Payment
Checks and cash are the accepted forms of payment. Payment is expected at the time of service unless otherwise arranged with the doctor. Delinquent payments of 60 days or more will be charged a $30 late fee. Any returned checks will receive a $30 service fee.
Release Form
In order to receive or transfer a copy of your chart, the Release Form located at this link must be completed, signed and returned to the office.
